This is a free consultation call - no diagnosis or treatment recommendation can be made without an evaluation. Please refrain from emailing personal health information or making medical inquiries over email, which is not HIPAA secure.

If you are a provider wishing to refer a patient, feel free to download and complete an optional Brooklyn Vision Therapy Referral Form and return via email or fax, or have your patient bring it in. Please fax any pertinent records, particularly from the evaluation that prompted referral. If you are an eye care professional, please fax their latest comprehensive eye exam notes. A written referral is not required, but will but allow us to send over the completed consultation report directly to your office. Patients may book online with or without a formal referral.
